Yes, Viva 3 is in network with most major insurance companies: Aetna, Blue Cross Blue Shield, Humana, Tricare, Medicare, Medicaid, United Healthcare, and UMR. We will work with you to determine if we are in network with your specific plan.
We do accept both PPO and HMO plans. Please note that HMO plans typically have a referral/authorization process that must be coordinated by your designated Primary Care Physician (PCP). If your referring provider is a different specialty, such as an orthopedic doctor, you are still typically required to have your PCP authorize your visit(s) with us.
Viva 3 is credentialed with Texas Worker's Comp and will help tailor a program for you to return to work.
Yes, we do accept cash pay. Please call for rates.
In some cases, you do not need a referral to see a physical therapist. However, some insurance plans may still require a referral or authorization before beginning treatment. We recommend calling so we can assist you in obtaining one or providing more information for your case.
